Scheduling & Payment Policy

• A $20 scheduling fee per dog is required to book an appointment.

• This fee holds your spot and goes toward your total grooming cost.

• The $20 fee is nonrefundable if you cancel, do not show, or are unhappy with results.

• Rescheduling does not incur a new fee if done within 24 hours and rebooked within 14 days.

• Payment for grooming is due at pickup. Accepted: Cash, Credit/Debit, or Zelle.

• Returned checks are subject to a $35 fee.

Health & Behavior Disclosure

I certify that my pet is current on rabies vaccinations and is in good health for grooming.

I will inform Gemstone Grooming of any health, skin, or behavioral issues (allergies, anxiety, aggression).

If my pet becomes unsafe to groom, services may stop, and partial fees may apply.

Matted Coat Policy

• Mats can cause skin irritation or discomfort.

• Dematting may be painful or require shaving for comfort and safety.

• Gemstone Grooming is not liable for irritation, nicks, or redness from mat removal.

Fleas, Ticks, or Parasites

If fleas or ticks are found, a flea/tick bath will be performed and a $15–$25 sanitation fee may apply.

Medical Emergencies

If my pet becomes ill or injured, I authorize Gemstone Grooming to seek immediate veterinary care.

I assume full responsibility for any treatment or transport costs.

Media Release

I DO OR I DO NOT

give permission for Gemstone Grooming to photograph or video my pet for marketing purposes.

Liability Release

I understand that grooming may expose pre-existing conditions. I release Gemstone Grooming and its staff

from liability for reactions or issues occurring during or after grooming. Every effort will be made to ensure

my pet’s comfort and safety.

Agreement
